Service Agreement

1.This agreement is made on _________________________ between _______________________________________(client) and Baker Surveillance & Investigation Services further referred to as (B.S.I.S.)

2. In consideration of the agreements made herein, B.S.I.S. will conduct investigative activities consisting of:

3. Said investigative activities will begin on or about _______________________.

4. B.S.I.S. bills fees off a retainer, The client will pay a Fee Retainer of Case
Specific prior to commencement of
services. B.S.I.S. accepts Visa, Master Card, American express, and Checks.

5. The client agrees that additional activities of B.S.I.S. (Charlie Baker)
will be paid by the client at the rate of $125.00 per hour for one employee.
B.S.I.S. charges $500.00 per day for Court appearances and depositions plus
travel per employee or agent, and lodging costs and in addition $100.00 per day
for essentials and $60.00 per hour for each additional employee, plus
applicable expenses as detailed above, and to include meetings, conferences, phone
calls, and emails. B.S.I.S. Charges a minimum of 5 hours on Surveillance
Assignments.

6. The client assumes responsibility to provide accurate information concerning
the investigation. B.S.I.S. Is not responsible for unproductive
investigative time resulting from inaccurate information supplied by the client
and there is no guarantees expressed or implied as to the outcome of any
investigation.

7. B.S.I.S. agrees that its
representatives will maintain reasonable communications with the client during
the period of the investigation. The client agrees that meetings and
conversations B.S.I.S.
representatives may be billed to the client at the rate of $125.00 per hour.

8. Unless waived herein by the
client, B.S.I.S. to provide,
within 10 days of conclusion of the investigation services contracted herein, a
written report accurately detailing the hours worked, activities fully
accounting for time charged, and results of the investigation. Any retainer balance shall be refunded by check no earlier then 7 days and no later then 14 days after case close date.

10. Client states under penalties for fraud that they have a legitimate legal
purpose for any information obtained by B.S.I.S.

11. Client agrees to hold B.S.I.S.
harmless for any lawful work done on said case assignment.

12. Client states under penalty for fraud that they are not under a restraining
order by subject of case assignment. Client States under penalty of perjury
they will use report provided by B.S.I.S. for legitimate legal purposes and
will not use information to stalk harass or disturb subjects peace.

14. This is authorization to act as our agent in the
above matter. We agree to indemnify and hold Baker Surveillance &
Investigation Services, and or any Agents working on their behalf, harmless
from and against any and all claims, damages, losses and actions including
reasonable attorney fees resulting from and arising out of our lawful efforts.
